What information will I need when I make a report?

  1. The child’s name, gender and approximate age
  2. The name of the person believed to be responsible for the abuse or neglect
  3. A description of the injury, neglect or threatened harm to the child
  4. The current location of the child; day care or school; home address
  5. Any immediate risk to the child
  6. The name of the person making the report and identifying information IF the caller wishes to give that information; anonymous reports are accepted and investigated.

What information will I need when I make a report?

  1. The name and address of the adult, or of any other person responsible for his/her care
  2. The age of the adult
  3. The nature and extent of the abuse, neglect, or exploitation, including any evidence of previous abuse, neglect, or exploitation
  4. The identity of the perpetrator, if known
  5. The identity of the complainant, if possible; and
  6. Any other information that the person believes might be helpful in establishing the cause of abuse, neglect, or exploitation.
